1. Data Collection Plan
Identify one primary instrument you will use to collect your project data (e.g., pre/post-test, survey, validated questionnaire, focus group protocol).
Ensure your instrument is previously validated in the literature—state the source.
Describe the type of data you will collect quantitative, qualitative, or mixed methods.
Explain how the tool aligns with your clinical question.
Identify the source of your data (e.g., patients, staff, medical records).
2. Measures and Data Collection Procedures
Describe how and when data will be collected.
Examples:
Quantitative: Pre/post-intervention metrics, readmission or infection rates, surveys.
Qualitative: Interviews, focus groups, journaling.
Describe your formative evaluation methods (e.g., weekly huddles, observation logs, checklists).
Describe your summative evaluation plan (e.g., measuring a reduction in falls, improvement in compliance, etc.).
3. Data Analysis Approach
Describe how you will analyze the data collected.
Include the statistical test(s) or qualitative methods you will use and provide a rationale.
Examples: Chi-square, t-test, Wilcoxon rank-sum test, content analysis.
Explain how the analysis supports answering your clinical question.
4. Privacy and Confidentiality
Describe how you will maintain confidentiality, privacy, and secure data storage.
Include methods such as encrypted devices, locked files, or secure servers.
