The goal of this assignment is for you to incorporate what you have learned in this course about  differential diagnoses and pathophysiology and apply the information to a case study of your choice. You may pick a family member, or a client from your practice setting experiencing a disease process and discuss the pathophysiology with data to support both differential and primary diagnoses. Please maintain patient confidentiality.  Use your pathophysiology textbook and textbook on differential diagnoses to complete this assignment. Present the patient via a SOAP note whereby: S = Subjective data, O = Objective data, A = Assessment (diagnoses) and P = Plan. The grading rubric for the case study is included in this syllabus. Include evidence (e.g., clinical practice guidelines) for ruling in or out differential diagnoses and choosing a primary or final diagnosis. Include patient education, anticipatory guidance, and recommendations for appropriate follow-up.  The management plan will be based on evidenced-based data or recommendations consistent with best practice guidelines. Recommendations from the Cochrane Database, National Guideline Clearinghouse, Clinical Guidelines and Evidence Reports from the AHRQ or a similar source should be used as evidence that the recommendations being presented represent the standard of care for the condition or disease that you are writing about.

Use 7^th ed. APA format for the cover page, running head, page numbering, in-text citations and the reference list. Otherwise, the paper will be written and typed as you would write a comprehensive SOAP note. You will need to cite from at least four (4) peer reviewed references published within the past five years outside of course textbooks. The paper should be a minimum of 5 pages, excluding the title page and references. Your written case study paper must be submitted via the Drop Box under assignments in the Folio Course (see course schedule for specific due date).
