Assignment:

You are the home care nurse assigned to visit an 85-year-old male patient who was discharged from the hospital with new-onset left-sided heart failure. You arrive at Mr. Smiths house to find that he is having difficulty breathing and appears anxious. You quickly assess Mr. Smiths lungs and find that he has bilateral rhonchi in the upper lobes. Mr. Smith shares that he has not taken his medications this morning. You ask to see the discharge instructions and note that Mr. Smith has seen several specialists, and each has prescribed a different medication regimen. You begin by determining which medications are due at this time.

2. What measures would be most effective to reduce adverse effects of medication administration?
3. How would the nurse ensure that the patient does not have duplicate medication orders and that the patient understands his medication regimen?
4. Explain to Mr. Smith what polypharmacy is and how to avoid it. How will you evaluate whether the teaching was effective?

Create a PowerPoint presentation that addresses each of the assignments points/questions. Be sure to completely answer all the questions for each bullet point. Use clear headings that allow your professor to know which bullet you are addressing on the slides in your presentation.

Assignment Expectations–

Length: 17-20 slides; answers must thoroughly address the questions in a clear, concise manner 

Structure:  

Title slide and reference slides in APA style. (at least 2 slides)

Scenario (11-14 slides)

Supporting articles (3-4 slides)

References (1-2 slides)

Additionally, because a good presentation has few words on the slides include a script with the verbiage you would say when presenting; script should be a minimum of 50 words per slide.

References: Use the appropriate APA style in-text citations and references for all resources utilized to answer the questions. Include at least three (3) scholarly sources/articles to support your claims.

Format: Save your assignment as a Microsoft PowerPoint document and a Word document (.pptx) and (.doc or .docx) or Open Office Impress. Make sure to reference the citations using APA writing style for the presentation. Include a slide for your references at the end. Follow best practices for PowerPoint presentations related to text size, color, images, effects, wordiness, and multimedia enhancements.
