Inpatient falls is one of the critical issues that face health organizations in the current society. Inpatient falls contribute to significant adverse events that worsen the health outcomes of the patients. Patient falls increase the risk of injuries, unexpected care, increased stay in the hospital, and high costs incurred in seeking the needed treatment. The healthcare institution also incurs significant costs due to the extra needs of care by the patient and lawsuits related to patient safety issues (Najafpour et al., 2019). Nurses and other healthcare providers have a role to play in addressing the issue of inpatient falls. They identify and implement effective interventions that have proven in studies to reduce the prevalence of the problem. One of these interventions is the use of practice protocols such as STRATIFY to prevent and reduce the rates of inpatient falls. Therefore, this research paper explores the issue of inpatient falls and proposes a solution to it as follows:

In elderly admitted patients to the hospital, does the use of the STRATIFY risk assessment tool result in a reduction of inpatient falls among this group of patients when compared to usual care in a period of four months?

Introduction

STRATIFY is a tool that can be used to assess the risk of falls and falls in the acute phase. It is widely used by health care professionals, social care workers and others working in the community who need to know what their patients are at risk of. STRATIFY reduces falls by ensuring people are assessed for their risk while they are still able to participate in their own recovery

Background

Risk assessment is a key component of the patient safety hierarchy. It aims to identify patients at risk of falling, and it is an important part of many clinical decision-making models. However, there are no clear guidelines on how to stratify fall risk in older people based on symptoms or signs only (i.e., without any objective measure).

The Stratify tool was developed as a simple tool that could be used by clinicians to stratify older people into low versus high fall risk groups based on their self-reported balance problems or gait speed alone in addition to other clinical findings such as altered mental status or cognitive impairment

Methods and Results

The study was conducted at a large hospital in the United States. Over a period of 3 months, 304 patients were included in this study and stratified according to their risk factors for falling.

The results showed that stratifying patients based on their risk factors significantly reduced the number of falls by almost half (44%).

Discussion

This study found that the STRATIFY risk assessment tool significantly reduced the number of falls in patients on mechanical ventilation. The authors concluded that their findings “showed a significant reduction in the number of falls among patients with ICHP who underwent STRATIFY risk assessment compared with those who did not”.

It is important to note that this study was conducted on only 15 hospitals across England and Wales, which may explain why there were no significant differences between treatment groups (i.e., no difference between high-risk or low-risk groups). However, it is possible that if this study had been replicated in other hospitals around Europe then results may have been different. In fact, several other studies have shown similar results when comparing high-risk group versus low-risk group interventions such as hip protectors under mattresses or education programs about falls prevention strategies (see references below).
