Based on the knowledge you have gained to this point, it is time to apply your patient education skills to a real-world scenario.

In this assessment, you will review a case study for a patient presenting with chronic knee pain. Using theory and evidence-based guidelines, you will convey your findings and care plan in a slide presentation.

Review the following scenario:

Your patient presents to the clinic today with a chief complaint of bilateral knee pain. He is unemployed and has state-funded health insurance.
• Age/sex/race: 63-year-old Native American male
• Height: 5’10”
• Weight: 230 lbs.
• BMI: 33
• BP: 144/88
• Pulse: 80
• O2: 99% RA
• Physical exam: Negative except for tenderness over bilateral knee pain with mild swelling, and pain with ROM. No evidence of instability in the joint.
• Review of systems: Positive for bilateral knee pain
• Imaging: X-ray done 1 month ago indicates “severe bilateral knee joint space narrowing in all compartments” noted with osteophytes
• Medical history: Seasonal allergies
• Surgical history: None
• Depression screen: Negative
• Medications: Naproxen, Excedrin PM
• Social: Married, lives in a two-story home with a bathroom and bedroom on the second floor, denies smoking, occasional alcohol use, denies current drug use (history unknown)

The patient reports a long history of chronic pain, mostly in his knees. Both knees seem to have recently worsened and are now consistently 4/10 on the pain scale. His pain is exacerbated by activity. It does not seem to radiate. He tried icing it and taking Tylenol with little relief and admits taking Ibuprofen occasionally when he “can’t take it anymore.” He experiences morning stiffness that improves with movement. His primary concern is persistent pain.

Assessment Deliverable
Based on the scenario provided, create a 15-slide presentation, with detailed speaker notes, in which you educate the patient about your findings and explain the diagnosis. Address the following in your presentation:
• Explain the pathophysiology of osteoarthritis (OA), rheumatoid arthritis (RA), and a gout flare in detail. Describe the risk factors for each condition. Cite at least 3 evidence-based sources to support your explanation.
• Identify the most likely diagnosis based on the patient’s history and symptoms. Provide a clear rationale for your conclusion.
• Develop a management and/or prevention plan that considers the patient’s cultural background, language, health literacy, and socioeconomic status. Include both pharmacological and lifestyle modification strategies.

Format citations and references for any sources used to support your case study according to APA guidelines.
