The write-up must include all of the following sections for full credit: 

(1) Background information: a detailed and specific picture of the background information of the fictional character is presented, including behavioral, social, academic, religious, and occupational functioning. Remember, if you can’t find details on some of these areas, you can feel free to present your own speculations about possible background information. 

(2) Crisis Issues and Potential Countertransference: The student clearly identifies or notes the status of each of the following potential crisis issues: risk of suicide based on assessment, history of child abuse and/or elder abuse, risk of harm to others, and history or current use/abuse of substances abuse; In addition, the student explains potential areas of therapist countertransference based on the client’s presentation.

(3) Diagnosis: diagnostic considerations are explored, including a tentative DSM-5 diagnosis based on clearly defined symptoms.  Student demonstrates a strong ability to talk through their decision-making process when considering various diagnoses.

(4) Treatment plan: a  clear and coherent treatment plan is presented that includes the presenting problem, long-term goals and short-term objectives, and interventions that correspond with the objectives; the student also discusses how to integrate the client’s culture and faith into treatment, and offers creative and evidenced-based solutions that connect the individual to community resources; Treatment plan is specific, measurable, and demonstrates an appreciation and knowledge of evidence-based treatment options.

(5) Write-up: case presentation is laid out with comprehensive content, is written professionally, free of grammatical errors, and is well-organized. The paper is at least 6 pages, including sections headings titled “Background Information,” “Crisis Issues and Potential Countertransference,” “Diagnosis,” and “Treatment Plan."
